AdviCare Disease Management
AdviCare Care Management can help your patients better understand and control chronic conditions, such as asthma, congestive heart failure and diabetes. The program encourages patients to take a more active role in self-care and emphasizes the importance of compliance with physician recommendations and treatment plans.
This voluntary program identifies eligible participants using predictive modeling software and claims data. Participants receive educational materials and personalized support from designated, trained health care professionals, including registered nurses and dieticians. While the frequency and methods of contact differ for various levels of member participation, AdviCare offers every participant medication adherence and pharmacy support, comorbidity management, behavioral health intervention, and health and wellness support. Easy-to-use tools and practical resources are designed to make the delivery of quality care easier.

CareEnhance
This nurse advice line program, administered through McKesson
Health Solutions, offers eligible members:
- Toll-free access to registered nurses utilizing:
- General health information
- Algorithm-based symptom assessment
- Appropriate care advice and follow-up as necessary
- Access over 1,200+ topics in our Audio Health Library
(500+ in Spanish)
- Assistance in making more informed health care decisions
24-hours a day, seven days a week
- Quarterly mailings to raise awareness and encourage
utilization of available services; includes refrigerator
magnets and wallet cards

Health Coach
This program provides one-on-one support for your patients
to set and reach goals for a healthier lifestyle. Participants
receive personalized support and educational resources
for health issues such as:

- And more – all with the focus of getting and
staying healthy
Health Coaches step participants through:
- Stages of change
- Assessing readiness
