pharmacy and behavioral health programs.
Disease management programs assist your patients
with:

- Diabetes
- Congestive heart failure (CHF)
- Coronary artery disease (CAD)
- Chronic obstructive pulmonary disease (COPD)
- Depression (managed as a comorbid condition)

| Regence Disease Management (DM) is staffed by RNs with
clinical experience in DM conditions. Additional qualifications
include:
- Certified Case Managers (CCM)
- Chronic Care Professionals (CCP)
- Certified Diabetes Educators (CDE)
In addition, nursing staff are supported by staff
physicians who are available for immediate consultation.
Disease management programs offer:

and following your care recommendations
- Education about chronic conditions so your patients
better understand the care plan you have recommended
Members who could benefit are identified, contacted
and encouraged to enroll in the programs. Enrolled
members receive tailored interventions, including:
- Newsletters
- Care reminders
- Clinical support
- Educational materials
- Other outreach as needed, based on unique individual
needs
