Regence Case Managers, who are experienced
registered nurses and social workers, provide
one-on-one support and guidance to your patient.
Under your directed treatment plan, case managers
support and guide each participant as best suited
to meet their specific needs.
Case Management is appropriate for:
- Patients of all ages, birth through seniors
- Patients with complex or catastrophic medical condition(s)
or event(s)
- Patients identified as needing additional support
and guidance through medical challenges
Rare Disease Condition Management Program
Statistically, one in ten Americans suffers from a rare and complex condition, creating a significant impact on health care costs. Regence Rare Disease Condition Management Program expands our care management services to include an intensified care management program for rare and complex conditions. Offered in partnership with Accordant®, this program addresses the complex challenges of patients with these conditions.
Our program supports your care plans for patients with rare conditions. The added support and your participation in the program helps strengthen your patient/physician relationship, patient adherence to care plans and early identification of intervention opportunities.
The program will focus on the following conditions:
- Hemophilia
- Polymyositis
- Scleroderma
- Cystic fibrosis
- Crohn's disease
- Gaucher disease
- Multiple sclerosis
- Dermatomyositis
- Seizure disorders
- Sickle cell disease
- Myasthenia gravis
- Parkinson's disease
- Rheumatoid arthritis
- Systemic lupus erythermatosus
- Amyotrophic lateral sclerosis (ALS)
- Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP)
Regence members who are eligible for the program will receive:
- Individualized treatment plans
- Personalized counseling and support
- Educational materials, including online resources and tools
The rare disease condition management services support physician strategies by:
- Providing assigned clinical staff to notify providers via telephone and fax of significant changes in his or her patients' health
- Providing nurse supported treatment regimens by developing individualized care management plans that encourage condition-appropriate behaviors, such as proper medication use and regular physician follow-up
