How many benefit plans are available within

Regence Select encompasses four distinct product
categories, for a total of 35 primary plan designs:
- RegenceEssentials, complete coverage that
is simple and affordable
- RegencePreferred, preferred coverage with
maximum choice and variety
- RegenceClassic, premium coverage with choice
and security
- RegenceClassic Plus, premium coverage for
ultimate security
Groups also have the ability to customize their
benefits by adding optional benefits and/or riders
such as:
- Rx Drug Card
- Vision
- Additional Accidental Injury
- Dental
- Personal Choice Account Services

Is the Integrated Behavioral Health Program
(IBHP) included in Regence Select? |

Yes. The IBHP provider network will be included
in each network option and Reliant Behavioral Health
will continue to coordinate with Medical Services
to provide behavioral health management services. |

subject to the benefit design chosen by the employer. |

How will immunizations be covered? |

Immunizations will be covered at 100%. Should
a provider charge for an office visit, it may be
subject to a copayment or deductible and coinsurance. |

What are the maternity benefits? |
| A: |
Maternity is an optional benefit for groups
with less than 10 enrolled employees. Groups of
10 or more enrolled employees will have maternity
included within the benefits.
When maternity coverage is provided, maternity
and conditions due to pregnancy for the enrolled
female employee or enrolled female spouse will
be paid according to the coinsurance amount for
physician and hospital services, subject to the
deductible. Involuntary complications of pregnancy
will be considered as any other illness. Routine
inpatient nursery room charges are payable following
birth for enrolled newborns, subject to the enrolled
child's deductible and coinsurance. Maternity,
including involuntary complications of pregnancy
are not provided for dependent children. |

Is there a difference between primary
and specialty definitions and copayment
levels? |

Yes. Primary care physicians practice in the
areas of family and general practice,
internal medicine, pediatrics, and OB/Gyn. Specialty
physicians specialize in areas of
medicine other than the primary physicians.
There are separate specialist co-payment on plans
that include the physician office copayments.
This amount is $15 more than the physician office
visit co-pay. |
