NOTE: Only available to residents of Asotin & Garfield counties in Washington state.


Rates

Rates may be paid by automatic bankdraft or through monthly billing. To pay by automatic bankdraft, your client should complete the Surepay Aurthorization.

Rates are set by your client's age on the date their coverage becomes effective. Rate changes made because of a change in age will take place on the anniversary date of the policy.

Waiting Periods for Preexisting Conditions

  • Benefits for preexisting conditions will only be provided for services  (3) three months or more after the Insured’s Effective Date under this Policy. The waiting period shall not apply to Newborn Children who shall be covered from the moment of birth, or to adopted children or children in the process of adoption who have been placed with the Policyholder, if such children are enrolled within sixty (60) days of birth, adoption, or placement. A pregnancy existing on the Effective Date of coverage will not be considered a Preexisting Condition. This waiting period shall not apply to coverage for PKU formulas.
  • Regence BSI shall waive any time period applicable to a Preexisting Condition exclusion or limitation period for the period of time an individual was previously covered by Qualifying Coverage, provided that the Qualifying Coverage was continuous to a date not more than sixty-three (63) days prior to the Effective Date of coverage under this Policy. The insured shall be allowed a credit against the Preexisting Condition exclusion or limitation period under this Policy for a Qualifying Coverage that the insured had at the time of application under this Policy, or that had terminated no more than ninety (90) days before the Insured applied, if the coverage was similar to this Policy.
  • Benefits for Human Organ and Tissue Transplants and Bone Marrow Reinfusion will only be provided after twelve (12) months of continuous coverage under this Policy subject to the limitations set forth in these General Provisions.

Limitations

  • Total outpatient benefits paid for treatment of Mental or Neuropsychiatric Conditions shall be limited to a maximum of $500 per insured each calendar year.
  • Total benefits paid for Inpatient treatment of Mental or Neuropsychiatric Conditions shall be limited to a maximum of $10,000 during a period of twenty-four (24) consecutive calendar months and a maximum of $30,000 during an Insured’s lifetime.
  • Total benefits paid for the treatment of Chemical Dependency shall be limited to a maximum of $10,000 during a period of twenty-four (24) consecutive calendar months.
  • Total benefits paid for Outpatient rehabilitation benefits, including physical, occupational, respiratory or speech therapy, shall be limited to a maximum of $1,000 per insured each calendar year.
  • Total benefits paid for Inpatient rehabilitation services shall be limited to $15,000 per insured each calendar year.
  • Benefits for treatment of temporomandibular joint (TMJ) disorders and orthognathic conditions shall be limited to a maximum of $2,000 paid during an insured’s lifetime.
  • Total benefits paid for covered human organ and tissue transplant and bone marrow reinfusion services shall be limited to a maximum of $150,000 during an insured’s lifetime.
  • Total benefits paid for home health care visits shall be limited to a maximum of one hundred thirty (130) visits per Insured each calendar year.
  • Total benefits paid for a routine eye examination shall be limited to a maximum or one examination per Insured each twelve (12) months.
