1. Open the file from where you saved it locally on your computer.
2. When the macro alert pops up, click “Enable Macro.” The calculator will not work unless macros are enabled.
3. Choose a family structure to quote.
4. Choose the subscriber’s gender.
5. Choose the subscriber’s age.
6. Choose a zip code prefix.
7. Enter a Health Factor then press “Enter” or “Tab” to calculate.
8. Click on the “Summary” tab to view or Click the “Print Summary” button in the upper right hand corner of the calculator.

The approximate monthly rates for all plans available (based on demographics entered) will appear on the summary page. If not, try resetting the form with the “Reset Form” button at the top of the form and try again.
