Terms and Conditions
The lifetime maximum for all Regence Chamber Select
plans is $1 million.
* The out-of-pocket maximum does not include deductible,
or any applicable co-pays.
** Networks subject to regional availability.
Rate structures between the three networks may not
remain constant in all regions. In areas where the
networks are not available, the TraditionalCare
network applies. Network numbers shown are as of
